For Region 92 matches, the referee is required to submit a report of any send-off offense to the Region 92 Referee Administrator within 48 hours.

For Area 1D U16/U19 and playoff matches, the referee is required to submit a report of any send-off offense to the Area Director within 48 hours.

NOTE: Cautions may be reported on the back of the lineup cards.

If you are injured during an AYSO event, you should fill out this form and contact the Safety Director.

This report should be filled out if a serious injury or violent incident occurs.

For Region 92 matches, you should forward your report to the Regional Referee Administrator and the Regional Commissioner.
