Misconduct Reports Referees are required to submit a Referee Misconduct Report of any send-off offense that occurs in a Region 92 U10/U12/U14 match to the Region 92 Referee Administrator within 48 hours. For Area 1D U16/U19 and U10/U12/U14 playoff matches, the referee is required to submit this report to the Area Director within 48 hours. NOTE: Cautions are to be reported on the back of the lineup cards.
Accident Reports If you are injured during an AYSO event, you should fill out an AYSO Accident Report Form and contact the Safety Director. Answers to frequently asked questions regarding soccer accidents may be found on the AYSO
administrators and management website.
Serious Incident Report An AYSO Incident Report Form is used whenever there is a personal injury, damaged property, or threats of or actual physical violence surrounding an AYSO game, practice, event or property. The form should be prepared by the coach, AYSO Official, or AYSO Volunteer which may be a member of the regional staff such as the regional safety director, or by tournament or event staff members. Instructions are included within the linked document. For Region 92 U10/U12/U14 matches, you should forward your report to the Regional Referee Administrator and the Regional Commissioner. For all Area 1D U16/U19 matches and U10/U12/U14 Area 1D playoff matches, you should forward your report to the
Area Director within 48 hours.
AYSO/USSF Cross-Certification Cross-Certification to USSF is available to AYSO Intermediate, Advanced or National referees who have been certified as referees for six months or more and who are not currently certified with USSF. All games officiated for AYSO or USSF may be applied toward the certification requirements of either AYSO or USSF. If you are interested in cross-certification and want to know more, contact
Region 92 Referee Administrator.

might help our volunteer referees improve their knowledge and skills. Please feel free to offer your
comments, be they praise and/or suggestions for improvement. After completion, please submit this form to the Region 92 Referee Administrator.
